DIXON FAMILY SERVICES JOB DESCRIPTION

Bilingual Case Manager, Part-time position (up to 20 hours per week)

 

QUALIFICATIONS:

 

  • High School Graduate, with some college courses in Human Services
  • Fluent in English and Spanish, verbal and written
  • Computer skills including data entry
  • Highly organized, self-motivated, able to work with little supervision
  • Ability to work with, and sensitive to, multi-cultural clients, volunteers, and staff
  • Able to motivate clients to bring out their highest potentials
  • Ability to keep accurate records for case files, tracking and reporting
  • Caring nature, with enthusiastic and positive attitude
  • Able to work in total confidentiality
  • Open to new ideas, trends and changes to maintain excellence in service delivery

 

RESPONSIBILTIES: 

  • With clients, complete comprehensive intakes, assessments and case plans
  • Provide linkages and navigation between clients and needed services – both at DFS and other agencies
  • With clients, set goals and action steps in their individual case plans
  • Schedule follow-up appointments with clients, as needed. Measure success, and determine next steps
  • Follow up with phone calls to clients who don’t show for appointments, address challenges and barriers
  • Keep current, precise and complete case files on all clients
  • Enter all client activities and communications in data base system including visits, info/referrals and other
  • Keep apprised of contract scopes of work, to fulfill requirements
  • Submit monthly reports as required by Director
  • Compile outcomes reports using vignettes, testimonials and success stories
  • Accept phone calls from people seeking human services
  • Mentor and train intern case managers
  • Participate in case management team meetings and case conference meetings as needed
  • Track and report client progress through outcomes, to report clients are healthier than when they began
  • Keep current on all available resources, up-date and expand resource lists, as needed
  • Attend all staff meetings and other meetings as required by Director
  • Perform other duties as assigned by Director

 

Pay range is $13 – $16.  Prorated Sick, Vacation and Holiday pay.  Rate for new hire depends on experience, skills and the agency’s budget.  No medical benefits are currently provided.

Dixon Family Services is seeking a part-time Bilingual Case Manager.  Join our team in providing basic needs and high-risk case management services for families with children in the community of Dixon.

 

Qualifications:  High School graduate, with some college courses in Human Services or previous experience helping families.  Fluency in both English and Spanish is required.  Computer skills including data entry also required.  Position is for 20 hours per week.

 

Brief Job Description: Complete intakes and comprehensive assessments with families struggling to meet basic needs.  Supportive activities include parenting education, navigation of benefit systems, budgeting, housing, food, employment development and other on-going services to assist in achievement of family stability.
